Make each requirement specific
List the supplier, the evidence you need, and the person responsible for reviewing it. Explain why your team is requesting the document and how you will judge whether it meets the requirement.
The included example rows are illustrative, not a universal list of legally required documents. Remove anything that does not apply and add requirements appropriate to your buyer, industry, and jurisdiction.

Keep evidence separate from the decision
Keep a reference to the original supplier document and write down the review outcome separately. Receiving a PDF, running AI screening, and accepting evidence internally are different steps.
Use review statuses consistently, such as awaiting evidence, needs review, or internally accepted. An internal status should not imply independent certificate authentication or legal certification.
Build a repeatable review routine
Review outstanding requirements regularly, follow up with the right supplier contact, and check upcoming review dates. When a replacement arrives, record what changed and reconfirm its dates before marking the requirement accepted.
